  • 18-Year-Old Student Parent/Guardian Authorization & Consent Form

    Students who have reached the age of 18 may choose to obtain access to verify their attendance after receiving written authorization from parents/guardians and obtaining approval from their Associate Principal.
  • Terms/conditions:

    • Parents/guardians acknowledge that they may no longer receive information about attendance and verification of all absences will be left up to their student. 
    • The privilege to authorize absences is limited to appointments that require students to leave during the school day. 
    • If students are absent due to illness, a written note should be turned into their Student Support Team office on the day they return to school. No phone calls or Skyward requests will be accepted from you or your parents/guardians. 
    • Consideration of this authorization will be given to students with a satisfactory record of attendance.
    • Once granted, this authorization is revocable if it is misused.
    • Failure to follow the WHS Student/Parent Handbook and/or a decrease in satisfactory attendance will result in revocation of this authorization. 
    • Discovery of a forged parent/guardian signature will result in an immediate revocation of this authorization and referral to the Student Support Team Dean of Students.
